Job Title: Intake Specialist

Community Care Alliance is seeking a compassionate and detail-oriented Intake Specialist to join our team. As an Intake Specialist, you will play a critical role in the development of comprehensive health service plans and daily organizational and treatment planning for our clients.

About the Role:
  • Develop and implement comprehensive health service plans for clients
  • Conduct daily organizational and treatment planning
  • Monitor client responses to health coordination and prevention activities
  • Collaborate with multidisciplinary teams to provide high-quality care
Requirements:
  • Bachelor's degree in psychology, social work, or related field preferred
  • AA degree or equivalent education level in human services required
  • One year of experience in community mental health preferred
  • Completion of CCSP training required
  • Reliable vehicle, auto insurance, and valid driver's license required
