About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Hospice team. As a LPN, you will play a vital role in providing high-quality patient care and supporting our patients and their families during a challenging time.

Responsibilities
  • Provide direct patient care, including pain and symptom management, to ensure the best possible quality of life for our patients.
  • Implement nursing practices based on comprehensive assessments and care plans, ensuring the highest level of patient satisfaction.
  • Document patient services accurately and in a timely manner, maintaining the highest standards of record-keeping.
  • Assess and educate patients and their families/caregivers on various aspects of care, promoting independence and empowerment.
  • Participate in the coordination of hospice services, ensuring seamless care delivery and patient satisfaction.
  • Manage pain and symptoms effectively, evaluating patient responses to treatments and medications.
  • Engage in professional growth and development opportunities, staying up-to-date with industry best practices and quality assessment activities.
  • Ensure compliance with all state and federal regulations, maintaining the highest standards of care and professionalism.
Qualifications
  • Graduate of an accredited practical or vocational nursing program, with current licensure to practice in the state of operation.
  • At least one year of nursing experience, with three or more years preferred.
  • Experience in community health, hospice, or med/surg settings is highly desirable.
  • Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
