**Job Title:** Medical Care Coordinator

**Location:** Amherst, MA

At Hilltown Community Health Center, we are seeking a highly motivated and detail-oriented Medical Care Coordinator to join our team. As a Medical Care Coordinator, you will play a crucial role in ensuring the smooth operation of our healthcare services.

**Responsibilities:**

  • Assist healthcare providers with patient care duties, including taking vital signs, preparing patients for exams, and maintaining accurate electronic health records.
  • Ensure that exam spaces are clean and ready for patients, equipment is sterilized, and supplies are stocked and available.
  • Utilize computer skills to enter information into electronic health records, prepare prescriptions, and assist with prior authorization.

**Requirements:

  • Graduate of an accredited Medical Assistant program
  • High school diploma or equivalent
  • Experience using computer-based systems to input data
  • Ability to maintain strict confidentiality of patient information in accordance with HIPAA and HCHC policy
  • Culturally qualified to work with diverse populations
  • Demonstrated commitment to electronic recording and providing others with accurate, detailed information
  • Bilingual (English-Spanish) strongly preferred
