Behavioral Health Community Health Worker

We are seeking a skilled Behavioral Health Community Health Worker to join our team at LifeLong Medical Care. As a key member of our Behavioral Health Team, you will play a vital role in providing direct care services to our diverse patient population.
Key Responsibilities- Facilitate and manage patient referrals to therapy and psychiatry, ensuring seamless integration of care.
- Screen, triage, and connect patients to internal and external behavioral health resources, maintaining an in-house waitlist as needed.
- Manage mental health provider schedules to optimize productivity and fill provider schedules.
- Conduct warm hand-offs of patients from medical providers to facilitate integration of care and connect high-risk patients to behavioral health services.
- Support the Behavioral Health Team by actively participating in behavioral health meetings and establishing a smooth functioning site.
- Track patients' progress, alerting providers to the need for outreach and enhanced services.
- Assist in coordinating and supporting behavioral health patient groups.
- Ensure appropriate consent and parental involvement for pediatric patients in behavioral health.
- Provide basic support to patients in connection with patient portal and video telehealth visits.
- Maintain updated resource lists of BH resources in the community and participate in special projects under the direction of the supervisor.
- Bachelor's Degree in Social Work, Health, or Human Services field or equivalent combination of education and/or experience.
- Administrative experience in a health or social service setting.
- Knowledge of East Bay health and social service resources.
- Previous work providing services to persons with mental health disorders, substance use disorders, physical health conditions, and/or disabilities, and/or who are experiencing homelessness.
- Proficient in Microsoft Office with ability to manage databases.
- Experience and sensitivity working with people who are low-income, have histories of trauma, have mental health and/or substance use disorders, and/or who are HIV positive.
- Epic experience.
- Bilingual English/Spanish/Arabic.
