We are seeking a highly skilled Behavioral Health Advocate Specialist to join our team at LifeLong Medical Care in West Berkeley. As a key member of our Behavioral Health Team, you will be responsible for facilitating and managing patient referrals to therapy and psychiatry, as well as connecting patients to internal and external behavioral health resources.

Job Summary

This is a full-time, benefit-eligible position that requires a Bachelor's Degree in Social Work, Health, or Human Services field or an equivalent combination of education and/or experience. The ideal candidate will have administrative experience in a health or social service setting and knowledge of East Bay health and social service resources.

Responsibilities
  • Facilitate and manage patient referrals to therapy and psychiatry, ensuring seamless integration of care.
  • Connect patients to internal and external behavioral health resources, maintaining waitlists as necessary.
  • Conduct warm hand-offs of patients from medical providers to facilitate care continuity.
  • Support the Behavioral Health Team in establishing and maintaining smooth functioning, actively participating in meetings.
  • Assist in tracking patient progress, alerting providers to the need for outreach and enhanced services.
  • Coordinate and support behavioral health patient groups, ensuring appropriate consent and parental involvement for pediatric patients.
  • Provide basic support to patients regarding patient portal and video telehealth visits.
  • Conduct data collection, entry, and report generation, participating in special projects under direction.

  • Ability to prioritize tasks, work under pressure, and complete assignments in a timely manner.
  • Excellent interpersonal, verbal, and written skills, with cultural sensitivity and ability to work with diverse backgrounds.
  • Commitment to working directly with low-income persons, providing helpful and supportive services.

Salary: $21-$22/hour
