Job Summary:

We are seeking an experienced RN - Home Health to join our team at Amedisys Home Health in Greenville, SC. As a Home Health Nurse, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to achieve optimal patient outcomes.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, focusing on promoting independence and improving quality of life.
  • Conduct thorough assessments, develop and implement individualized care plans, and evaluate patient progress.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care.
  • Document patient information accurately and timely, maintaining confidentiality and adhering to regulatory requirements.

Requirements:

  • Current South Carolina nursing license.
  • At least 2 years of recent Home Health US nursing experience as a RN - Home Health.
  • Current BLS and/or ACLS (AHA Preferred) and/or Specialty Certifications.
  • Active Nursing License per state.
