Key Responsibilities
  • Provide skilled nursing visits to home health patients, including admission, case management, and follow-up care.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Confer with physicians to develop and revise care plans, ensuring seamless communication and coordination of care.
  • Coordinate care with other healthcare professionals, including physical therapists, occupational therapists, and home health aides.
  • Document patient care and progress, ensuring accurate and timely reporting to physicians and Clinical Managers.
  • Supervise and oversee the work of home health aides and personal care workers, providing guidance and support as needed.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum of one year of nursing experience, with strong med-surg, ICU, ER, and acute experience preferred.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills, with ability to work independently and as part of a team.
