About the Role:
As a Home Health LPN, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Our team is dedicated to providing comprehensive home health services that promote independence, dignity, and well-being.
Key Responsibilities:
- Provide professional nursing services under the direction of a RN, adhering to state Nurse Practice Act, licensure/certification requirements, healthcare standards, governmental laws, and CenterWell Home Health policies.
- Deliver skilled nursing interventions to address patients' illness, rehabilitative needs, and preventative care.
- Utilize a holistic approach to provide problem-specific therapeutic interventions, teaching, and training activities in accordance with the plan of care.
- Collaborate with the RN Supervisor to ensure seamless care coordination and make necessary changes to the Plan of Treatment.
- Communicate effectively with patients, physicians, clinical managers, and other healthcare team members to ensure timely and efficient care.
- Participate in care coordination activities and discharge planning as needed.
- Maintain confidentiality and adhere to professional standards in the provision of care.
- Prepare and submit documentation as required by the Company and/or facility.
Requirements:
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
