Role Overview

Rehabilitation Associates is a leading organization in post-acute care, focusing on an interdisciplinary team approach. As a Primary Medical APP, you will play a crucial role in managing acute medical issues for short-term rehabilitation patients and chronic conditions for the long-term care population.

Key Responsibilities
  • Patient Assessment and Management: Conduct thorough patient examinations, obtain medical histories, and assess health status to provide personalized care.
  • Diagnostic and Therapeutic Services: Order, perform, and interpret diagnostic tests; prescribe medications and manage treatment plans to achieve optimal patient outcomes.
  • Collaboration and Consultation: Work closely with physicians and other healthcare professionals to coordinate patient care, consult on complex cases, and develop effective treatment strategies.
  • Patient Education and Support: Provide guidance on preventive health care, manage patient education, and offer support to enhance understanding of treatment plans and promote patient empowerment.
  • Triage and Communication: Handle patient calls, evaluate concerns, and triage effectively to ensure timely medical responses and address patient needs promptly.
Work Environment

This role offers a supportive work environment that prioritizes work-life balance. Key benefits include no call duties, a flexible schedule allowing a four-day work week, and daytime responsibilities from 8 AM to 5 PM only. Remote documentation flexibility provides further convenience.

Requirements
  • Certification: Active Physician Assistant or Nurse Practitioner License (Required)
  • Experience: Minimum two years of experience as a PA/NP, preferably in an acute, subacute, or rehabilitation setting.
Compensation and Benefits

We offer a competitive salary range of $135,000 - $170,000 per year, along with comprehensive benefits including 401(k) matching, health insurance, short and long-term disability insurance, life insurance, and generous paid time off.
