Key Responsibilities:
RN Role Summary:
The primary responsibilities include coordinating patient care with the interdisciplinary team, which encompasses physicians, patients/families, and referring agencies.
Deliver skilled nursing care across all age groups in the patient's residence while taking charge of care coordination.
Essential Functions of the RN:
- Manage cases and provide clinical guidance to the interdisciplinary team, physicians, and families.
- Develop, implement, and document the patient care plan.
- Administer care while adhering to infection control protocols that safeguard both staff and patients (OSHA standards).
- Ensure the continuity of high-quality patient care with appropriate documentation.
- Monitor assigned cases to verify compliance with third-party payer requirements.
- Exhibit commitment to professional development and competency.
- Uphold agency philosophy and administrative policies.
- Perform on-call duties and provide on-call support to patients and families as required.
- Supervise and guide home health aides and LPNs.
Qualifications:
RN Job Requirements:
Education:
- Graduate from an accredited Diploma, Associate, or Baccalaureate Nursing Program.
Licensure:
- Current State Registered Nurse license and a valid Driver's License.
Experience:
- Minimum of two years of experience as a Registered Nurse in a clinical care environment, preferably in home health or hospice settings.
Skills:
- Nursing skills consistent with widely accepted standards of practice. Strong interpersonal abilities. Proof of CPR certification, Hepatitis consent/declination, and Covid Vaccination or eligibility for a religious/medical exemption.
Transportation:
- Reliable transportation and valid auto liability insurance.
