About the Role

As a Hospice Case Manager at Corewell Health, you will play a vital role in providing compassionate end-of-life care to individuals with a life expectancy of six months or less. Our team is dedicated to delivering high-quality, patient-centered care in the comfort of their own homes, nursing facilities, or in the hospital.

Key Responsibilities

  • Collaborate with the interdisciplinary team to assess, plan, implement, coordinate, monitor, and evaluate strategies and services to meet patients' and families' end-of-life care needs.
  • Develop and maintain outcome-based plans of care, utilizing the nursing process, relationship-centered care principles, and a holistic philosophy to ensure the changing needs of patients and families are met.
  • Under the supervision of the Nursing Manager, use clinical judgment skills to access and implement treatment plans for patients, assessing, monitoring, detecting, planning, and taking action to prevent physiological complications associated with specific health problems and/or medical treatment.

Qualifications

  • Associate's degree in Nursing required.
  • Bachelor's degree in Nursing required within five years.
  • Michigan Registered Nurse (RN) license required.
  • AHA or ARC Basic Life Support (BLS) certification required within 90 days.
  • Driver's license within the State of Michigan.
