As a RN Case Manager Hospice at Corewell Health, you will play a vital role in providing end-of-life care and symptom management to individuals with a life expectancy of six months or less. Our team is dedicated to delivering compassionate and patient-centered care, and we are seeking a skilled and empathetic RN to join our interdisciplinary team.

Key Responsibilities
  • Assess, plan, implement, coordinate, monitor, and evaluate the strategies and services required to meet a patient/family's end-of-life care needs.
  • Collaborate with the interdisciplinary team to create an outcome-based plan of care that addresses the changing needs of the patient/family.
  • Use clinical judgment skills to access and implement the treatment plan for patients, ensuring a symptom-free and peaceful death.

Qualifications
  • Associates in Nursing Required.
  • Bachelors in Nursing Required within 5 years.
  • MI Registered Nurse (RN) Required.
  • AHA or ARC Basic Life Support (BLS) Required within 90 days.
  • Driver's License within the State of Michigan.
Work Environment

This role is based at our Continuing Care E Beltline location in Grand Rapids, Michigan, and requires a full-time commitment with a 40-hour workweek. The shift is day-time, with hours of work from 8 a.m. to 4:30 p.m., Monday to Friday, with on-call weekends.
