About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team at Haven Hospice. As a Registered Nurse, you will play a vital role in providing high-quality care to patients and their families in their homes and communities.

Responsibilities
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by the physician.
  • Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions between healthcare settings.
  • Communicate effectively with patients, families, and healthcare providers to ensure that care is patient-centered and meets the unique needs of each individual.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellence in patient care.
Requirements
  • Current Registered Nurse licensure in the state of practice.
  • One year of experience in hospice or palliative care preferred.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Basic computer skills and proficiency in electronic medical records.
