Job Summary

Key Responsibilities
  • Develop and implement personalized care plans to meet the unique needs of our members
  • Collaborate with interdisciplinary teams to coordinate member services and ensure timely access to care
  • Provide emotional support and education to members and their families, empowering them to take control of their health
  • Advocate for members and navigate the healthcare system to ensure seamless care transitions
  • Manage and resolve administrative issues, ensuring compliance with hospital and legal requirements
Requirements
  • Minimum of 1 year experience in case management, member care, customer service, or a related field
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Proficient in computer skills, including documentation, email, and chat support
  • Ability to work independently and collaboratively as part of a team
  • Strong organizational and time management skills, with the ability to multitask and prioritize tasks effectively
