At MedCentric, we are dedicated to enhancing the quality of life for older adults facing complex health challenges. We are looking for a committed Registered Nurse to deliver exceptional, integrated care as part of our PACE (Program of All-Inclusive Care for the Elderly) interdisciplinary team.

In this position, you will oversee and provide direct nursing services to PACE participants across diverse environments, including clinics, care centers, residences, virtual consultations, skilled nursing facilities, and more. As an essential member of the interdisciplinary team, you will work collaboratively with other healthcare professionals to perform thorough assessments, create tailored care plans, and ensure prompt service delivery.

This role offers the opportunity to practice at the peak of your professional capabilities, fostering continuous learning and the advancement of your nursing skills. You will have the ability to customize the care experience, focusing on the unique needs and preferences of each participant.

Key Responsibilities:

  • Act as the primary liaison for the interdisciplinary team, ensuring progress on participants' care strategies
  • Administer nursing care in various environments, including primary care, medication management, wound care, oxygen therapy, and more
  • Perform comprehensive nursing evaluations of physical, psychosocial, and behavioral conditions
  • Recognize and independently initiate emergency interventions as necessary
  • Educate participants, caregivers, and staff on individualized care plans and response techniques
  • Monitor and assess therapeutic interventions, suggesting modifications to enhance health and wellness
  • Provide on-call support as required

Ideal Candidate:

  • Genuine commitment to serving high-risk seniors and frail older adults
  • At least 2 years of experience caring for adults, particularly those who are chronically disabled or elderly
  • Ability to operate independently in managing clinical needs and program functions
  • Strong active listening and conflict resolution abilities
  • Flexibility to adapt to various clinical environments
  • Training in wound care, IV administration, and blood draws
  • Current, unrestricted RN license in California
  • BSN preferred
  • CPR and First Aid certification
  • Alzheimer's certification
  • Proficiency in English and Spanish is preferred
