About the Job

CenterWell Home Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care to patients in need. As an LPN, you will play a vital role in delivering compassionate and personalized care to patients in the comfort of their own homes.

Job Responsibilities
  • Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitation, recovery, and prevention.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for addressing these needs.
  • Collaborate with the RN Supervisor to ensure seamless care coordination and communication with patients, families, and healthcare teams.
  • Document patient progress, including visit notes, treatment plans, and other relevant information, in accordance with CenterWell Home Health's documentation standards.
Requirements
  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
