Job Title: Rehabilitation Case Manager - Brain Injury

Overview:

Learning Services, A Collage Rehabilitation Partner is seeking a highly skilled and compassionate Rehabilitation Case Manager to join our team in Gilroy, CA. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional neurological rehabilitation to adults with brain injury.

Responsibilities:

  • Develop and implement individualized treatment plans, ensuring seamless coordination of care from admission through discharge and follow-up.
  • Coordinate schedules, therapies, and appointments, ensuring timely and effective care.
  • Establish and maintain regular communication with families and funders, addressing their concerns and needs.
  • Collaborate with the medical director and external medical providers to ensure optimal medical care and coordination.
  • Work closely with the program nurse and clinical director to ensure clinical and medical needs are met.
  • Communicate with the Operations Director to ensure residential care is being met and personal preferences are identified.
  • Prepare and maintain accurate clinical documentation, including report writing and facilitating funding issues as needed.
  • Act as a client advocate, ensuring their rights and interests are protected.

Qualifications:

  • A bachelor's degree in a related field, with case management experience in the healthcare industry or rehabilitation.
  • A master's degree and CRC or CCM designation are highly desirable.
  • A valid driver's license and a record of responsible driving behaviors.
  • Excellent planning, organizational, multitasking, writing, and time management skills.
  • Strong clinical skills, including the ability to manage crisis situations, assess client needs, and develop treatment plans.
  • Excellent communication and interpersonal skills, with the ability to work effectively with families, professionals, and team members.
