Advanced Nurse Practitioner

Greater Seacoast Community Health is seeking a highly skilled Family Nurse Practitioner to join our team of Family Medicine Providers delivering Primary Care services to patients and families. As a Family Nurse Practitioner, you will carry a panel of Adult and Pediatric patients seeking Primary Care services at Goodwin Community Health in Somersworth, New Hampshire.
Key Responsibilities:
- Provide clinical services to patients in accordance with approved treatment protocols and recognized standards of medical practice.
- Engage with clinical and nonclinical services available to patients and Providers within the federally-designed "patient-centered medical home" model of care.
- Coordinate care and deliver treatment plans to patients with complex care needs, or at risk of developing complex care needs.
Requirements:
- Must hold a Bachelor's Degree in Nursing from an accredited university program.
- Graduate of an accredited Advance Practitioner Registered Nursing program.
- Active, unrestricted certification by the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
- Must hold current, unrestricted Registered Nurse (RN) license and Advanced Practice Registered Nurse (APRN) license administered by the New Hampshire Board of Nursing.
- Active, unrestricted federal DEA registration and New Hampshire DEA registration, or eligible for federal and NH registration.
Preferred Qualifications:
- 0-5 years' experience.
- Ability to work harmoniously with coworkers and patients.
- Experience working with diverse and vulnerable populations.
- Excellent communication and listening skills.
- Organizational skills.
- Strong clinical and assessment skills.
- Compassionate and nonjudgmental attitude.
