Job Summary:

Greater Seacoast Community Health is seeking a highly skilled Family Nurse Practitioner to join our team of Family Medicine Providers delivering Primary Care services to patients and families. As a Family Nurse Practitioner, you will carry a panel of Adult and Pediatric patients seeking Primary Care services at Goodwin Community Health in Somersworth, New Hampshire.

Key Responsibilities:
  • Provide clinical services to patients in accordance with approved treatment protocols and recognized standards of medical practice.
  • Engage with clinical and nonclinical services available to patients and Providers within the federally-designed "patient-centered medical home" model of care.
  • Coordinate care and deliver treatment plans to patients with complex care needs, or at risk of developing complex care needs.

Requirements:
  • Must hold a Bachelor's Degree in Nursing from an accredited university program.
  • Graduate of an accredited Advance Practitioner Registered Nursing program.
  • Active, unrestricted certification by the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
  • Must hold current, unrestricted Registered Nurse (RN) license and Advanced Practice Registered Nurse (APRN) license administered by the New Hampshire Board of Nursing.
  • Active, unrestricted federal DEA registration and New Hampshire DEA registration, or eligible for federal and NH registration.

Preferred Qualifications:
  • 0-5 years' experience.
  • Ability to work harmoniously with coworkers and patients.
  • Experience working with diverse and vulnerable populations.
  • Excellent communication and listening skills.
  • Organizational skills.
  • Strong clinical and assessment skills.
  • Compassionate and nonjudgmental attitude.
