Nurse Practitioner or Physician Assistant

We are seeking an experienced Nurse Practitioner or Physician Assistant to join our team at Salmon Falls Family Healthcare in Somersworth, NH.
About the RoleThe Primary Care Provider will be responsible for diagnosing and treating patients in a kind and compassionate, care-centered, outpatient environment.
Key Responsibilities- Assessment, diagnosis, prescription, selection and administration of therapeutic measures and treatment regimes
- Consult, plan and implement collaborative management, referrals, or transfer of care as appropriate
- Practice within standards consistent with those established by the New Hampshire State Board for his/her license
- Maintain up to date and timely records and documentation
- Allocate sufficient time as is reasonably necessary for effective patient care, including direct patient care and administrative/documentation work

Requirements- Candidate must be licensed or able to obtain a license in the State of New Hampshire
- Candidates seeking part and/or full-time positions will be considered
- Pediatric experience is a plus as we are seeking additional support on our Peds team
