Advanced Practitioner Primary Care Provider

We are seeking an experienced Nurse Practitioner or Physician Assistant to join our team at Salmon Falls Family Healthcare in Somersworth, NH.
About the RoleAs a Primary Care Provider, you will be responsible for diagnosing and treating patients in a kind and compassionate, care-centered, outpatient environment. Your duties will include:
- Assessment, diagnosis, prescription, selection and administration of therapeutic measures and treatment regimes
- Consult, plan and implement collaborative management, referrals, or transfer of care as appropriate
- Practice within standards consistent with those established by the New Hampshire State Board for your license
- Maintain up to date and timely records and documentation
You will work closely with our team to provide a variety of services, including consultation and referral, routine office visits, medication management, and collaboration and support across provider group.
RequirementsTo be successful in this role, you will need:
- Licensure or ability to obtain licensure in the State of New Hampshire
- Experience in pediatric care is a plus, as we are seeking additional support on our Peds team
- Ability to work flexible hours and maintain confidentiality as it applies to all state, federal and company guidelines
