Job Title: Bilingual Case Manager

Join our team at Berkshire Farm Center & Services for Youth as a Bilingual Case Manager, where you will play a vital role in supporting the well-being of children and families in need.

Job Summary:

The Bilingual Case Manager will be responsible for assessing the needs of Unaccompanied Alien Children (UACs) in care, developing Individual Service Plans, and ensuring the safe and timely release or discharge of children and youth. This role requires strong communication skills, cultural competence, and the ability to work effectively with diverse populations.

Key Responsibilities:
  • Maintain positive partnerships with referring agencies, coordinating required services for youth and families, and ensuring timely and effective communication regarding case activities.
  • Provide crisis intervention services for youth experiencing acute distress and dysfunctional behaviors to address safety concerns.
  • Offer supportive case work counseling to youth and families, including assessments, treatment, and discharge plans.
  • Coordinate psycho-educational groups as needed.
  • Schedule and facilitate home visits that are goal-oriented and work towards building trust, communication, and healthy relationships.
  • Conduct comprehensive treatment reviews according to agency and state regulations.
  • Complete all state-mandated and agency documentation according to policy and regulatory requirements.
  • Comply with federal, state, and local laws and regulations, including those of ORR, ICE, and the NYS Office of Children and Family Services.
  • Ensure a safe and therapeutic environment by attending internal and external meetings and case reviews.
  • Prepare court reports and attend court proceedings as scheduled and required.
Requirements:
  • Bachelor's degree in the behavioral sciences, human services, or social services fields.
  • Minimum 2-4 years of child welfare and/or case management experience is strongly encouraged.
  • Fluent in English/Spanish with excellent verbal and written communication skills.
  • Current and valid driver's license with an acceptable driving history.
  • Ability to work a flexible schedule, including night and weekend hours as necessary.
  • Varicella Vaccine or titers indicating immunity.
  • MMR (Measles Mumps & Rubella) vaccine or titers indicating immunity.
  • Tdap (Tetanus-diptheria-pertussis) vaccine.
  • Influenza vaccine, one dose annually during flu season.
  • Physical (Medical Exam) required.
  • Ability to work with economically and culturally diverse populations.
Compensation:

$25.95 hourly

Schedule:

Sunday - Thursday, 9 am to 5 pm
