Job Summary

We are seeking a highly skilled and compassionate HIV Mental Health Specialist to join our team at Albany Medical Center. As a key member of our healthcare team, you will provide comprehensive mental health care coordination services to patients with HIV/AIDS.

Key Responsibilities:

  • Provide screening, intake, and brief mental health needs assessments for patients referred by healthcare team members.
  • Develop mental health referral plans and initiate referrals to external mental health service providers.
  • Complete referral paperwork, including consents for release of HIV and mental health information.
  • Follow up on referrals to ensure smooth transitions to mental health services.
  • Develop, implement, and update service plans for patients receiving mental health care coordination services.

Requirements:

  • Masters degree in Social Work, Counseling/Psychology, or related field.
  • One to three years of experience in case management and/or counseling services to medically compromised patient populations.
  • Familiarity with DSM 5 and community mental health resources.
  • Ability to work in a fast-paced, interdisciplinary team setting.
