Job Summary

The Care Manager-HIV Specialist monitors the health and utilization patterns of members living with HIV, providing support and assistance to those who need it.

Key Responsibilities
  • Monitor, track, and report viral load data for members with HIV
  • Build relationships with local care providers and community partners to expand lab data sharing
  • Act as the clinical resource for HIV campaigns and related messaging
  • Provide member/community education on HIV and other chronic health conditions
  • Coordinate care for members who meet criteria for case management across the continuum of care
Requirements
  • Registered Nurse with a current New York State license or Masters in Social Work with a current New York State License and certification
  • Associate degree or two (2) years of equivalent experience
  • Certified Case Manager (CCM) certification preferred
  • Minimum of three (3) years' clinical experience
  • HMO or managed care experience preferred
