Position Overview: The MDS Coordinator is tasked with overseeing and managing the execution of Minimum Data Set (MDS) assessments, ensuring adherence to all regulatory standards. This position plays a crucial role in maintaining precise and prompt documentation, collaborating with a multidisciplinary team, and aiding in the formulation of personalized care strategies.

Primary Duties:
  • Supervise and manage the execution of MDS assessments, including Admission, Quarterly, Annual, and Significant Change evaluations.
  • Guarantee that MDS documentation is precise, timely, and compliant with both federal and state regulations.
  • Work closely with the interdisciplinary team to collect and evaluate resident information for accurate MDS coding.
  • Design and implement care strategies based on MDS assessments and the specific needs of residents.
  • Continuously monitor and assess care strategies to ensure they accurately reflect the current status and requirements of residents.
  • Provide training and guidance to nursing and clinical personnel regarding MDS-related procedures and documentation.
  • Analyze MDS data to identify trends, outcomes, and opportunities for quality enhancement.
  • Prepare and submit MDS-related reports and documentation as mandated by regulatory bodies.
  • Engage in care strategy meetings, resident evaluations, and other interdisciplinary team initiatives.

Requirements:
  • Registered Nurse (RN) with a valid license in New York or a Licensed Practical Nurse (LPN) with relevant experience.
  • Preferred certification as an MDS Coordinator (e.g., RN-BC or equivalent).
  • Prior experience in long-term care and MDS coordination is essential.
  • In-depth knowledge of MDS assessments, care planning, and regulatory standards.
  • Exceptional organizational, communication, and analytical abilities.
  • Capability to work autonomously and collaboratively within a multidisciplinary team.
  • Familiarity with electronic health records (EHR) and MDS software.
