Job Summary

The MDS Coordinator will be responsible for coordinating and managing the completion of the Minimum Data Set (MDS) assessments in compliance with regulatory requirements. This role involves ensuring accurate and timely documentation, collaborating with the interdisciplinary team, and supporting the development of individualized care plans.

Key Responsibilities:
  • Oversee and coordinate the completion of MDS assessments, including Admission, Quarterly, Annual, and Significant Change assessments.
  • Ensure MDS documentation is accurate, timely, and in compliance with federal and state regulations.
  • Collaborate with the interdisciplinary team to gather and review resident data for accurate MDS coding.
  • Develop and implement care plans based on MDS assessments and resident needs.
  • Monitor and review care plans to ensure they reflect the current status and needs of residents.
  • Provide training and support to nursing and clinical staff on MDS-related processes and documentation.
  • Review and analyze MDS data to identify trends, outcomes, and areas for quality improvement.
  • Prepare and submit MDS-related reports and documentation as required by regulatory agencies.
  • Participate in care plan meetings, resident assessments, and other interdisciplinary team activities.
Qualifications:
  • Registered Nurse (RN) with a current and valid license in New York or a Licensed Practical Nurse (LPN) with relevant experience.
  • Certification as an MDS Coordinator preferred (e.g., RN-BC or equivalent certification).
  • Previous experience in long-term care and MDS coordination required.
  • Strong knowledge of MDS assessments, care planning, and regulatory requirements.
  • Excellent organizational, communication, and analytical skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Proficiency in electronic health records (EHR) and MDS software.
