Job Summary

The Public Health Nurse-Home Visitation Specialist is responsible for delivering high-quality education and healthcare services to families under the Bright Family Beginnings Program. This grant-funded position involves providing clinical case management, health promotion, and education to clients on various prevention-based concerns.

Key Responsibilities
  • Provide screenings virtually and via home visits to women and their families eligible for evidence-based programs
  • Carry a maximum caseload of up to 18-22 families and assist them in establishing goals and outcomes with educational supports
  • Develop and maintain cooperative working relationships with first responder agencies, local community partners, and stakeholders
  • Perform initial intake assessments and conduct appropriate health screenings for clients
  • Assist clients in enrolling in state assistance programs and provide education on family planning and contraception
Requirements
  • Knowledge of infant and child development, principles and practices of professional nursing, and health promotion and health prevention
  • Ability to work independently and make nursing decisions within the scope of practice
  • Effective communication and interpersonal skills, with the ability to interact with diverse populations
  • Ability to remain professional and courteous in high-pressure situations
Preferred Qualifications
  • Experience working in a public health setting and knowledge of community health improvement initiatives
  • Ability to work in a team environment and maintain confidentiality
  • Knowledge of Microsoft Office and basic internet skills
