Nurse Practitioner or Physician Assistant

Landmark Health, a part of Optum, is seeking a Nurse Practitioner or Physician Assistant to join our team in Saginaw, MI. Our medical group provides home-based medical care to patients, focusing on delivering high-quality, patient-centered care in the comfort of their own homes.
As a member of our interdisciplinary team, you will work collaboratively with other healthcare professionals to manage complex patient panels, providing comprehensive care and support to patients with diverse needs. Key Responsibilities:
- Provide high-quality, patient-centered care to patients in their homes, focusing on prevention, early intervention, and management of chronic conditions.
- Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to develop and implement patient care plans.
- Conduct comprehensive assessments of patients, including medical history, physical examination, and laboratory tests, to identify and address health needs.
- Develop and implement individualized care plans, including medication management, wound care, and other interventions as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient satisfaction.
- Participate in ongoing education and training to stay current on best practices and evidence-based care.
Requirements:
- Current state RN license and NP/PA license.
- Completion of a Nurse Practitioner or Physician Assistant accredited program.
- Access to reliable transportation.
Preferred Qualifications:
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
- Knowledge of CPT, ICD-10, and HCPC codes.
- Knowledge of clinical standards of care.
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
