Job Overview

CareSTL Health is in search of a dedicated Referral Coordinator to manage administrative responsibilities, provide recommendations for individuals seeking services, and communicate essential information to various organizations.

Key Responsibilities:

  • Facilitates both internal and external referral authorizations while monitoring specialty care referrals to identify patterns in patient adherence to provider recommendations, ensuring timely access to care.
  • Supports hospitals and insurance providers to secure referral authorizations promptly, aiming for optimal health outcomes for patients and their families.
  • Actively engages in performance improvement initiatives and collaborates effectively with insurers and healthcare facilities.
  • Generates monthly reports detailing the status of all referrals and diligently tracks patient referrals to external agencies, maintaining a comprehensive referral log within the electronic medical record (EMR).
  • Coordinates services from multiple agencies to enhance client support, assisting patients in navigating potential healthcare-related challenges, including financial or social barriers.

Additional Responsibilities:

  • Establishes connections among service providers and demonstrates the ability to work independently.
  • Encourages client accountability in maintaining appointments.
  • Assesses client progress in accessing necessary medical care and services.
  • Keeps technical knowledge and skills current.

Qualifications:

  • Education: High School Diploma or GED. Completion of an accredited Medical Assistant program is preferred.
  • Experience: 1-2 years in a community health center or medical office environment.
  • Other: Current CPR Certification is required. Valid registration as a Registered Medical Assistant in Missouri is preferred. Reliable transportation is necessary.

Additional Information:

  • Clinic operates Monday through Friday from 8 a.m. to 5 p.m.
