In the role of Clinical Assessment Coordinator, you will be instrumental in providing your team with the expertise needed to address each resident's unique requirements. Your responsibilities will include developing, monitoring, auditing, and refining personalized care plans to maintain the high standards of care. This encompasses conducting resident evaluations and assisting with the discharge process. The residents depend on your knowledge, skills, and attention to detail to ensure their health and comfort.
To qualify for this position, candidates must possess a valid and unrestricted State of Illinois R.N. License, have graduated from an accredited nursing program, and hold C.P.R. Certification. While prior experience as a Clinical Assessment Coordinator, one year as an R.N. in a Skilled Nursing Facility, and one year as a C.N.A. are beneficial, we are willing to invest in the right candidate.
