Job Title: Physical Therapist Assistant for Home Care

Alternate Solutions Health Network LLC is seeking a skilled Physical Therapist Assistant to join our team in providing high-quality home care services.

Job Summary:

We are looking for a dedicated and compassionate Physical Therapist Assistant to work with our patients in their homes, providing skilled physical therapy services under the direction of a Physical Therapist.

Responsibilities:
  • Provide comprehensive skilled services to patients in their homes, including physical therapy procedures and diagnostic tests.
  • Assess functional problems and develop interventions to improve patient functional status.
  • Communicate effectively with physicians, therapists, and coworkers to ensure seamless care coordination.
  • Participate in case conferencing and contribute to the development of patient care plans.
  • Document all aspects of treatment, assessments, and patient education in accordance with agency protocol and Medicare/Federal guidelines.
Requirements:
  • Current licensure as a Physical Therapist Assistant in the state of employment.
  • Minimum of one year of experience as a Physical Therapist Assistant in an acute setting.
  • Home care experience is desired.
  • Ability to effectively communicate and create positive impressions with patients, families, physicians, and coworkers.
  • Knowledgeable on therapy best practices.
  • Ability to make appropriate physical therapy judgments.
  • Ability to identify situations and handle them with the best possible solution.
  • Detail-oriented and observant.
  • Disciplined style of work ethic with the ability to prioritize and be timely.
  • Valid driver's license and auto insurance with your name as a listed driver.
