Job Title: Speech Language Pathologist Home Health

Alternate Solutions Health Network LLC is seeking a skilled Speech Language Pathologist to join our team in Chillicothe, Ohio.

About the Role:

We are looking for a compassionate and experienced Speech Therapist to provide home-based skilled speech therapy care for patients. As a Speech Language Pathologist, you will work closely with patients and their families to develop personalized treatment plans and improve communication skills.

Responsibilities:
  • Develop and implement individualized speech therapy plans for patients.
  • Conduct assessments and evaluations to identify communication needs and develop strategies for improvement.
  • Provide one-on-one therapy sessions to patients in their homes, focusing on swallowing, breathing, cognition, speech, and communication skills.
  • Collaborate with healthcare teams to ensure seamless care and effective communication.
  • Document patient progress and outcomes, adhering to agency protocols and Medicare/Federal guidelines.
Requirements:
  • Current license to practice as a Speech Therapist in the state of employment.
  • Minimum one-year experience as a Speech Therapist in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Ability to meet the physical demands of the job.
