DaVita is seeking a dedicated and compassionate Case Management Coordinator to join our team. As a key member of our Patient Pathway team, you will serve as a liaison between patients, physicians, hospitals, clinics, and healthcare insurance providers.

Key Responsibilities:

  • Intercept and respond to patient requests and concerns
  • Provide patient education on medical insurance and healthcare options
  • Collaborate with healthcare teams to ensure seamless patient care
  • Utilize strong customer service skills to build trust with patients and families

Requirements:

  • High School Diploma or equivalent required; Bachelor's Degree preferred
  • Intermediate computer skills and ability to learn new hospital systems
  • Strong work ethic and desire to serve patients
  • Knowledge of medical insurance (Medicare/Medicaid/Commercial) highly preferred
  • Valid driver's license and ability to travel locally
