Case Management Coordinator

About the Role:
We are seeking a highly skilled and compassionate Case Management Coordinator to join our Patient Pathway team at DaVita. As a key member of our team, you will serve as a liaison between patients, physicians, hospitals, clinics, and healthcare insurance providers.
Key Responsibilities:
- Intercept and respond energetically to the requests of multiple constituents
- Utilize knowledge base in patient education, including medical insurance, to connect with patients face-to-face
- Deal effectively with unique and sometimes difficult situations
- Work in an independent, goal-oriented environment
- Witness positive impact in the lives of patients daily

Requirements:
- High School Diploma or equivalent required, Bachelor's Degree preferred
- Intermediate-level computer skills and ability to learn new hospital systems
- Strong customer service skills and ability to work in face-to-face settings
- Valid driver's license and ability to travel locally
