- Monitor and report on quality indicators to drive continuous improvement.
- Conduct regular visits with clients to assess their needs and provide support.
- Current California State license as a Registered Nurse.
- Valid California Driver's License.
- Eligibility for liability insurance and a clean DMV record.
- Proficiency in software programs, including Microsoft Office.
- Excellent communication, problem-solving, and interpersonal skills.
- Experience in home health or hospice care.
- Discharge planning expertise.
- At least one year of work experience as an RN.
This role is based in Sacramento, serving the Sacramento and surrounding counties. The successful candidate will work in our office, with occasional travel to clients' homes.
Compensation and BenefitsWe offer a competitive hourly rate of $43 to $53, with opportunities for advancement and professional growth. Our benefits package includes health insurance, dental insurance, vision insurance, life insurance, and a 401(k) matching program.
