Hospice Clinical Liaison Specialist

We are seeking a highly skilled Hospice Clinical Liaison to join our team at Compassus. The successful candidate will be responsible for modeling our company values and promoting our philosophy, while upholding our Code of Ethical Conduct.
Key Responsibilities- Assist referral sources in identifying patients with care needs that match our hospice services.
- Communicate with patients, family members, and significant others to explain our services and benefits.
- Obtain signatures on required forms and ensure all necessary documents are completed and submitted.
- Establish and maintain professional relationships with community, hospital, and facility representatives.
- Facilitate patient admissions and discharges, and coordinate transportation, DME, and medication needs.
- Oversee the GIP patient admission process and provide education to hospitals and facilities on general inpatient care.
- Associate or Bachelor's degree in Nursing highly preferred.
- Two to three years of related experience required.
- Hospital and/or long-term care clinical experience highly preferred.
- Experience in healthcare marketing preferred.
- Strong written and verbal communication skills.
- Ability to read, analyze, and interpret general business periodicals and technical procedures.
- Ability to understand and speak English.
- Active and unencumbered Registered Nurse license, or Licensed Practical/Vocational Nurse, in state(s) of employment required.
The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities.
Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds.
Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected.
Routine use of standard medical equipment and tools associated with clinical care is essential.
