The Clinical Social Services Specialist offers essential support to patients and their families through various services including counseling, emotional assistance, crisis management, and resource linkage.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:
  • Graduation from an accredited institution in Social Work or a related discipline.
  • Must hold a valid license through the appropriate regulatory board with at least an LSW Provision.
PREFERRED QUALIFICATIONS:

EXPERIENCE:
  • A minimum of two years of experience in a healthcare or community service environment.


CORE DUTIES AND RESPONSIBILITIES:
The following responsibilities outline the general nature of the work performed by individuals in this role. This list is not exhaustive and additional duties may be assigned.

  • Support the mission of the organization to address the healthcare needs of the community.
  • Emphasize quality service delivery to both patients and staff.
  • Communicate effectively and respectfully with colleagues, especially in discussions of differing opinions.
  • Facilitate the progression of care plans by removing obstacles.
  • Connect patients and families with suitable hospital and community resources.
  • Assist in the development of complex discharge plans.
  • Collaborate with case management for ongoing care support as needed.
  • Participate in scheduled meetings to discuss patient care.
  • Document and report on quality metrics related to referrals.
  • Engage in self-assessment and create a professional development plan aligned with personal and organizational goals.
