Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at Mercy. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional outcomes.

Responsibilities
  • Assess, plan, implement, and evaluate patient care needs and outcomes
  • Provide skilled nursing services to patients in their homes, under the direction and supervision of a registered nurse
  • Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans
  • Conduct thorough assessments of patients' physical, emotional, and social needs
  • Develop and implement effective care plans to address patients' needs and promote optimal health outcomes
  • Provide education and support to patients and families to promote self-care and independence
  • Document patient care and outcomes in a timely and accurate manner
Requirements
  • Current LPN license in the state of Missouri
  • One year of experience in a related field (preferably in home health or hospice care)
  • BLS CPR Certification through American Heart Association
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to provide good written documentation in a timely manner
