Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality patient care in the comfort of their own homes. As a member of our home health team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Provide high-quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other members of the healthcare team.
  • Continually observe and report the patient's nursing needs to the supervising RN.
  • Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues or needs.
  • Document patient visits thoroughly and completely, maintaining an accurate and current medication listing.
  • Actively participate in the performance improvement plan, working to reduce patient hospitalizations and improve outcomes.
Requirements
  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
