Position Overview

ROLE SUMMARY:
The Senior Care Physician (PCP) serves as a leader within our collaborative care model. We emphasize value-based care, prioritizing quality for the patients we serve.

Our care team includes physicians, advanced practice professionals, pharmacists, care associates, medical assistants, behavioral health specialists, quality coders, referral coordinators, and more.

This integrated approach enables us to offer an unparalleled experience for seniors.

Our model is designed to enhance care quality and improve outcomes for seniors through a concierge experience, multidisciplinary services, coordinated care supported by analytics, and strong community ties.

This comprehensive support allows our PCPs to manage fewer patients, dedicating more time to patient care.
Key Responsibilities

PRIMARY DUTIES:
- Assess and treat patients in accordance with established care standards.
- Facilitate coordination of patient services, including specialty referrals, hospital and skilled nursing facility coordination, durable medical equipment, and home health services.
- Actively participate and provide medical expertise within the care team during daily meetings.
- Collaborate with the Lead Physician and Center Administrator to foster a cooperative practice environment through professionalism and approachability.
- Complete daily medical record documentation, working with quality coders to ensure comprehensive records.
- Adhere to policies and protocols set by Clinical Leadership.
- Meet regularly with the Lead Physician to discuss care quality, review outcome data, and address medical record concerns.
- Focus on direct patient care, balancing patient-facing time with necessary administrative tasks (charting, meetings, etc.).
Qualifications

ESSENTIAL QUALIFICATIONS:
- Graduate of an accredited MD or DO program.
- Current and unrestricted medical license or willingness to obtain one in the state of practice; eligible and willing to secure licenses in other states as needed.
- Board Certification or eligibility for certification (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
- Active and unrestricted DEA license.
- Medicare and Medicaid Provider Numbers.
- Strong interpersonal and communication skills with colleagues and patients.
- Fully engaged in a team-based care model.
- Willingness to learn and adapt to a value-based care environment.
- Excellent patient/customer service skills.
- Basic computer proficiency, including email and electronic medical records.
This position involves direct patient interaction. Selected candidates will undergo TB screening.

PREFERRED QUALIFICATIONS:
- Familiarity with HEDIS/STARS metrics.
- Understanding of CAHPS and HOS.
- Experience in medically underserved or culturally diverse communities.
- Proficiency with electronic health records.
- Experience in Medicare Advantage Global Risk Models.
