Lakewood Health System is seeking a compassionate and skilled LPN to join our Assisted Living team. As an LPN in Assisted Living, you will provide person-centered care to our residents, enhancing their quality of life and promoting dignity and independence.

Responsibilities

  • Provide clinical assistance to ensure resident safety and care
  • Ensure team members adhere to LHS clinical standards
  • Participate in care conferences to share insights and recommendations
  • Administer and manage medications according to resident plans of care
  • Monitor residents' health conditions and document interactions and care updates

Requirements

  • Graduate of an accredited school of nursing
  • Current LPN licensure in the State of Minnesota
  • Senior care experience preferred
