Position Summary:

Under the general oversight of the Program Manager, this role involves responsibilities related to the comprehensive care of clients and supervision of staff within designated residential facilities.

The candidate must successfully fulfill all functions and competencies associated with this position, as well as other duties as assigned.

Key Responsibilities:

Maintain regular communication with the Program Manager to ensure program quality

Adjust work hours as necessary for optimal program operation

Ensure adherence to relevant licensure regulations


Supervise and guide staff; instruct staff on acceptable work standards and identify when they are not meeting Agency expectations.

Provide active treatment, emotional support, and health-related services, including administering medications, assisting with breathing treatments, monitoring vital signs, and aiding with medical equipment

Conduct and document physical assessments, including taking and recording vital signs and medical histories

Assist with basic personal care and grooming, including bathing, hygiene, hair care, and dressing, as well as other support for clients

Monitor HPC forms and ensure compliance with goal data sheets. Ensure they are completed accurately.

Report UIR and MUI situations to the Program Manager following established guidelines

Ensure that ISP's and Behavior Support Plans are implemented and adhered to by all staff.

Train staff on all ISP provisions and collaborate with the Program Manager to discuss necessary adjustments.

Ensure adequate staff coverage at all times. Arrange for coverage or fill in for absent staff with authorized personnel to maintain full staffing.

Verify that assigned staff complete their mandatory monthly training within the designated timeframe.

Participate in and assist staff in delivering appropriate active treatment for clients

Oversee life safety training, emergency procedures, evacuation drills, and consumer participation in the proper use of the facility

Coach staff and collaborate with the Program Manager to implement appropriate corrective actions

Manage the handling of consumer funds as indicated in the ISP and per regulatory requirements, as delegated

Ensure all documentation is accurate and that the facility is always prepared for surveys


Perform routine building maintenance tasks such as window cleaning, pressure washing, changing light bulbs, yard upkeep including snow removal, lawn care, and flower bed maintenance.

Maintain a clean and safe environment at all times.

Complete administrative tasks related to facility operations as directed by the Program Manager

Transport clients in Agency and/or personal vehicles

Engage in direct care for 20 or more hours per week and administer medications.

Perform any other duties deemed relevant to position responsibilities by management

Requirements:
Minimum of a High School Diploma or GED and 2 years of Direct Care experience, or experience working with individuals with developmental or intellectual disabilities; preferably in a Lead or supervisory capacity; or a combination of experience and education beyond the high school level

Successfully pass a background check that includes a BCI and FBI criminal history check

Exceptional verbal and written communication skills are essential

Must maintain current Med Pass and CPR/First Aid Certification; training available for CPR and Med Pass onsite

A valid Ohio Driver's License with fewer than 6 points is required

Proficiency in Microsoft Office and other electronic devices such as computers, iPads, and smartphones
