About This Role

At St. Matthew's House Inc, we are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing support and guidance to residents in our Emergency Housing/Shelter. Your primary responsibility will be to assist residents in achieving self-sufficiency through advocacy, goal setting, and accessing community resources and support services.

Key Responsibilities
  • Collaborate with residents to develop and implement individualized case plans.
  • Assist residents in completing necessary paperwork and referrals.
  • Establish and track progress towards goals in areas such as obtaining identification, employment, education, and financial stability.
  • Maintain accurate and up-to-date case notes and records.
  • Refer residents to external agencies and resources as needed.
  • Work with volunteers to provide life skills classes and support.
  • Ensure the facility is maintained in a safe and secure manner.
  • Perform other duties as assigned by the Assistant Manager or Program Director.
Requirements

To be successful in this role, you will need:

  • A high school diploma or equivalent.
  • A minimum of 1 year of experience working with individuals who are homeless or struggling with addiction.
  • Strong communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
