Female-Only: Young Parent Program Support Specialist

The Young Parent Program Support Specialist will play a crucial role in delivering direct assistance and case management to participants in the program. This role includes maintaining accurate daily records for residents and providing support to the Program Director. All responsibilities must align with established program policies and procedures. Please note that this position is exclusively open to female candidates.
Key Responsibilities
- Engage effectively with youth and their families in dynamic environments, including transitional care and aftercare outreach.
- Facilitate the intake process for young parents, ensuring timely communication and provision of the program's Welcome Packet, which outlines expectations and requirements.
- Coordinate transportation for clients to the program and ensure all intake documentation is completed promptly.
- Conduct a thorough review of the Intake Safety Plan and Mental Health Status with a clinician shortly after intake.
- Maintain comprehensive records in compliance with Caring Together, EEC, and Youth Services standards.
- Develop and update Treatment Plans, ensuring necessary signatures and assessments are completed within specified timeframes.
- Organize treatment team reviews and ensure all documentation is current and signed as required.
- Perform CANS assessments and update Safety Plans regularly to reflect current needs.
- Utilize Virtual Gateway for documenting Treatment Plans and incident reports for DCF clients.
- Oversee the management of client binders, ensuring all documentation is organized and signed appropriately.
- Participate in team meetings and collaborate with various stakeholders to provide updates on young parent services and treatment goals.
- Support young parents in achieving a high quality of life, focusing on essential areas such as hygiene, nutrition, and living conditions.
- Contribute to program initiatives aimed at preparing adolescents for adulthood, covering topics like parenting skills, job readiness, and financial management.
- Ensure weekly group sessions are scheduled to enhance safety and independent living skills.
- Act as a positive role model for young parents and staff, demonstrating leadership qualities.
- Maintain communication logs and provide necessary updates regarding clients and programs.
- Participate in the Manager On Call rotation and document relevant communications.
- Complete all required assessments and forms in accordance with state regulations.
- Manage program fees and maintain accurate financial records for clients.
- Conduct training sessions for staff on various topics related to program operations.
- Facilitate monthly meetings for young parents, documenting agendas and attendance.
- Assist young parents in navigating housing applications and maintaining a Housing Binder.
- Participate in the interview process for new hires and interns, ensuring compliance with HR procedures.
- Provide transportation for youth under 18 to essential appointments.
- Engage in Continuous Quality Improvement (CQI) activities, presenting necessary information in meetings.
A Bachelor's degree in psychology or a related field is required, along with a minimum of two years of experience in a residential setting. The Young Parent Program Support Specialist must be flexible in working hours to accommodate client needs. Candidates should demonstrate strong communication and organizational skills and be committed to participating in diversity, equity, and inclusion training initiatives.
