  • Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure comprehensive patient care.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
  • Provide education and support to patients and their families on disease management, symptom control, and end-of-life care.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Active RN license in the state where the assignment is located.
  • 2+ years of experience in hospice care, with a focus on direct patient care.
  • HomeCare HomeBase experience required.
  • BLS certification with AHA.
