Social Worker

We are seeking a skilled Hospital Social Worker to join our team at MultiCare Health System. As a Hospital Social Worker, you will play a vital role in providing cross-continuum care coordination, psychosocial assessments, and interventions to patients and their families.
Responsibilities:- Perform cross-continuum care activities, including clinic, provider practice, urgent care, phone care agency, skilled care, or other sites of care, as appropriate.
- Provide customer service to patients, families, physicians, and care team members, ensuring seamless transitions of care.
- Model and orchestrate consistent, professional communications and maintain proactive communication patterns with colleagues and staff.
- Delegate responsibilities appropriately while remaining accountable for outcomes.
- Ensure patients and families experience medically appropriate transitions of care.
- Master's degree in Social Work.
- Licensed in the state of Washington.
- Licensed Independent Clinical Social Worker (LICSW) or Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain LICSW within four years.
- Minimum three years of Hospital or equivalent experience.
- Health industry knowledge preferred.
- Healthcare financial assistance knowledge preferred.
- Leadership experience a plus.
