CommonSpirit Health is seeking a highly skilled and experienced Care Coordination Director to join our team. As a key member of our clinical operations team, you will be responsible for planning, directing, and evaluating the full scope of care coordination and utilization review services.

Key Responsibilities
  • Plan, direct, and evaluate the programs, systems, operations, and resources of the care coordination function.
  • Develop departmental goals and objectives that align with the organization's strategic plan.
  • Prepare and manage the annual budget, ensuring effective use of resources.
  • Direct all aspects of patient care coordination within the clinical areas, ensuring seamless transitions and optimal patient outcomes.
  • Collaborate with the discharge planning team and external stakeholders to ensure appropriate hospital utilization, medical necessity, and care coordination processes.
  • Develop, implement, and evaluate new or revised policies, procedures, guidelines, training materials, and communications in support of the care coordination function.
  • Produce and present reports on key performance indicators and benchmarking metrics to inform strategic decision-making.
  • Conduct research, analysis, and assessment of current strategies and practices to identify areas for improvement and optimize results.
  • Recruit, develop, and manage staff, ensuring they have the necessary skills and competencies to excel in their roles.
Requirements
  • Oregon RN license or LCSW/MSW certification.
  • 5 years of demonstrated care management experience in an acute care setting, with 3 years of director-level experience and direct staff reports.
  • Required: Bachelor's Degree in a related discipline; Master's Degree strongly preferred.
