The ideal candidate will possess strong interpersonal skills, organizational abilities, and a deep commitment to improving patient outcomes through effective care coordination.Why Choose Umpqua Health?Impactful Work: Make a difference in our community by helping members navigate their healthcare needs.Supportive Environment: Join a collaborative team committed to your success and professional growth.Comprehensive Benefits: Enjoy competitive pay, medical/dental/vision insurance, and opportunities for advancement.Innovative Culture: Contribute to ongoing process improvements and technological advancements in healthcare service delivery.Your ImpactServe as a primary point of contact for patients, caregivers, and healthcare providers to facilitate smooth transitions of care.Conduct initial assessments to identify patient needs, goals, and barriers to care.Develop personalized care plans in collaboration with multidisciplinary healthcare teams and patients.Coordinate appointments, referrals, and diagnostic tests to ensure timely access to healthcare services.Advocate for patients to ensure they receive necessary treatments, medications, and supportive services.Educate patients and caregivers about their health conditions, treatment plans, and self-management strategies.Monitor patient progress and outcomes, adjusting care plans as needed to optimize health outcomes.Document all patient interactions, care plans, and interventions accurately in electronic health records (EHR) systems.Collaborate with insurance providers and community resources to address social determinants of health and facilitate access to supportive services.Participate in care conferences and meetings to discuss patient progress, goals, and discharge planning.Perform other duties as assigned by management needed to help drive to our Vision, fulfill our Mission, and abide by our Organization's Values.Your CredentialsHigh school diploma or equivalent. One-year healthcare experience.Certified Community Healthcare Worker (CHW) or become certified within first year of employment.Proven experience in care coordination or patient navigation within a healthcare setting.Strong understanding of medical terminology, healthcare processes, and patient care management.Excellent interpersonal and communication skills, with the ability to interact effectively with patients, families, and healthcare professionals.Organizational skills and attention to detail to manage multiple patient cases efficiently.Compassionate and patient-centered approach to healthcare deliveryUmpqua Health is an equal opportunity employer that embraces individuals from all backgrounds.
