About the Role

As a Care Coordination Navigator, you'll play a critical part in guiding patients through the complexities of the healthcare system, ensuring seamless transitions between care settings, and promoting comprehensive, patient-centered care.

Key Responsibilities
  • Serve as a primary point of contact for patients, caregivers, and healthcare providers to facilitate smooth transitions of care.
  • Conduct initial assessments to identify patient needs, goals, and barriers to care.
  • Develop personalized care plans in collaboration with multidisciplinary healthcare teams and patients.
  • Coordinate appointments, referrals, and diagnostic tests to ensure timely access to healthcare services.
  • Advocate for patients to ensure they receive necessary treatments, medications, and supportive services.
  • Educate patients and caregivers about their health conditions, treatment plans, and self-management strategies.
  • Monitor patient progress and outcomes, adjusting care plans as needed to optimize health outcomes.
  • Document all patient interactions, care plans, and interventions accurately in electronic health records (EHR) systems.
  • Collaborate with insurance providers and community resources to address social determinants of health and facilitate access to supportive services.
  • Participate in care conferences and meetings to discuss patient progress, goals, and discharge planning.
Requirements
  • High school diploma or equivalent.
  • One-year healthcare experience.
  • Certified Community Healthcare Worker (CHW) or become certified within first year of employment.
  • Proven experience in care coordination or patient navigation within a healthcare setting.
  • Strong understanding of medical terminology, healthcare processes, and patient care management.
  • Excellent interpersonal and communication skills, with the ability to interact effectively with patients, families, and healthcare professionals.
  • Organizational skills and attention to detail to manage multiple patient cases efficiently.
  • Compassionate and patient-centered approach to healthcare delivery.
