Job Summary: We're seeking a skilled Nurse Care Coordinator II 12Hr to join our team. In this role, you will conduct patient assessments, develop and implement plans of care, and collaborate with other healthcare professionals to ensure high-quality patient outcomes.

Salary Range: $99,424.00-$171,506.00

Key Responsibilities:
  • Conduct thorough patient assessments to identify needs and develop individualized plans of care.
  • Collaborate with interdisciplinary teams to coordinate care and achieve optimal patient outcomes.
  • Maintain accurate and up-to-date patient records, including progress notes and treatment plans.
