About the Position

We are seeking a skilled Mental Health Care Coordinator II to join our team at APAIT. As a key member of our interdisciplinary team, you will provide mental health services to clients, including case management, crisis intervention, and linkage to community resources. Your excellent communication skills and ability to work effectively with diverse populations make you an ideal candidate for this role.

APAIT offers a comprehensive range of services including individual and group mental health counseling, substance use prevention and treatment, harm reduction, and syringe exchange programs. Our commitment to positively impacting the quality of life for vulnerable communities drives our work.

Estimated Salary: $70,000 - $79,000 per year

Responsibilities
  1. Assess client needs and develop individualized treatment plans
  2. Provide mental health services, including case management, crisis intervention, and linkage to community resources
  3. Collaborate with interdisciplinary teams to coordinate care and services
  4. Document client progress and maintain accurate records in accordance with DMH guidelines
